 Programs at IU are approved by the Utah State Approving Agency to participate in GI Bill/VA Education Benefits. If you are a veteran or other eligible person, IU will help you through the process to receive all benefits for which you qualify. Proof of Eligibility:
To obtain proof of eligibility, Submit Form 22-1990 Application for Education Benefits to the Muskogee regional VA office. Doing so will generate a Certificate of Eligibility. Remember to retain a copy of the application for your records.

22-5490 Application for DEA benefits (6 pages) (Adobe PDF) If you have already applied and used your benefits at another institution, you will need to fill out Form 22-1995 Change in Program or Place of Training and submit to the Muskogee regional VA office. Submitting Proof of Eligibility and Official Transcripts to IU
IU will need a copy of the form that you choose to use as proof of eligibility. You will also need to submit official transcripts for ALL institutions previously attended. These transcripts include, if applicable, an American Council on Education transcript for any military education and experience. Transcripts and copies of your eligibility forms should be sent to:
